  Rare 'n Nirv Shoes Collection has one almost exactly like your second example. There's another PA who does a lot of more outrageous shoe/boot designs, but I don't immediately remember who.

  edit: I was thinking of dx30, but their products are almost all spike heels, rather than platforms.
