Buttons falling off clothes

I regularly have an issue with buttons falling off clothes. I do a render and suddenly notice a button floating in thin air. 
Q: Do you know why this happens?

    What items? Are they dForce clothes with the buttons separate items parnted to Rigid Follow Nodes (look in the Scene pane, expand the entry for the figure anfd then the clothes - which will usually be parented to the figure - and hen e xpand the clothes; are there entries under that node that look like buttons, and if you expand them have the buttons parented to them)?

    Adding to what Richard asked, are they non-dForce clothes to which you applied the dForce dynamic surface? That definitely causes floating buttons for me.
