So, I'm modeling a railing that'll have lots of these ugly belusters.
My questions are:
1. Would you keep them separate/unwelded from the base and handrail or extrude either?
2. should i uv before duplicating?

I'd keep them separate - easier to manage and keeps the polycount down.
Definitely uv-map first, then duplicate (you can always separate the individual uv islands later if you need to).
Is there some sort of array function in Carrara? Create one, use lots; and yes unwrap first and consider keeping separate.
(An array add-on (as seen in Blender and elsewhere), duplicates items at pretermined distances; straightlines, curves etc. Very useful if available and much easier to maintain. All modelling done on the original affects all; it is a form of instancing.)
I dunno about an array function in Carara's model room. Thereb is one in the assembly room, but that just makes instances of an object that'll only be visible in a render.

Yes you can replicate in the vertex room too and you can define the xyz parameters of the replications and distance
