That's how smoothing works. If you set clothing to collide with the skin, it doesn't know which bits of skin you want it to collide with, so when you've got skin-on-skin contact, or near to that, it will collide with BOTH bits of skin.
Using a dForm or Push Modifier to separate the touching parts may help.
