Anyone Know Where this Mirrored Crystal is From?

looks like some sort of mirror tile shader on random props actually
I'd try a sphere primative with a mirror shader and smooth turned off on the surfaces tab. (It's towards the bottom)
It doesn't look mirrored so much as it looks like it has very high refraction like a diamond. Use the Iray Uber glass with dispersion (rainbow) and change the refraction from 1.55 to something like 2.40. If it doesn't have enough colors in it for your taste, lower the abbe value by .5 increments (the default for that glass is an abbe value of 55.00). First try 50.00, then 45.00, etc. until you get something you like. Or, you could just use the Iray Uber diamond material ;). Up to you.
And yes, turn off smoothing.

Edit: the image is a pair of earrings I made-the ball has smoothing turned off and the material is the glass shader with the refraction set to 2.40 and the abbe set to 45.00. The round corners radius is .0025 so that you can see the thin edges of the facets (because nothing in nature is razor sharp). The larger the object is, the more you will have to increase this value.
Now that I look at the stuff in the promo image again, the carafe looks like one of the items from Pauli's Bride's Gold (a very old item). :)
