Try lowering the Glossy Roughness and/or Top Coat Rougness (in Surfaces tab [Iray]) below .20 until you get the effect you want.
You could try this (select the "Skin" surfaces):
Metallic Flakes Weight: 0.05
Metallic Flakes Roughness: .45 (Play with this setting. Lower is shinier)
Metallic Flakes Size: 0.01
Metallic Flakes Strength: 0.5
Metallic Flakes Thin Film: 10000
Metallic Flakes Thin Film IOR: 1.0
Other metallic flakes settings at default.
You can turn off limits and increase the glossy layered weight and the top coat weight values. The values to use would depend on the light and the other parameters on the skin. You could try increasing these numbers in integer increments until you get the desired look.
V3Digitimes's Iray Converter products include a set of gloss management scripts that give you an easy way of tuning the gloss on skin/lips/nails as well.
That looks like a high top coat weight with a low roughness on it.
You might try tweaking the "Glossy Layered Weight", "Glossy Specular" color, and "Glossiness" to see if that gives you the look you want BEFORE resorting to messing with the "Metallic Flakes Weight". Increasing the Metallic Flakes Weight will often increase your render time by like 200%.
I usually just add a geoshell, apply the iray uber base, then apply the iray water preset.
